I hereby certify that my child is fully capable of participating in the designated sport and is healthy and has no physical or mental disabilities that would restrict my child’s full participation in these activities, except as noted above. In addition to giving my full consent for my child’s participation, I hereby waive, release, and hold harmless MSYSA and all participating schools and sponsoring organizations, their officers, coaches, sponsors, supervisors, and representatives for any injury that may be suffered by my child in the normal course of participation in the designated sport and activities incidental to this. Parent/Guardian Signature (PRINT) Date Let’s Play! – FUNdamentals Pledge The following expectations are very important to us and help make our program a safe place, both physically and emotionally, for all players. We call them our FUNdamentals, because we want them to be the foundation for every player to have FUN while with us. We ask every player and family to be familiar with these expectations before starting and agree to abide by them. I, Child’s name for pledge, would like to participate in the Let’s Play! afterschool soccer program. I understand that the following rules are expected to be upheld and that failure to do so could result in my suspension and/or expulsion from the program. Have Fun! – Because if you’re not having fun, YOU’RE NOT HAVING FUN! Respect Yourself and Others – so we can all feel safe and supported while having fun. No name calling No swearing No destroying property No fighting (keep your hands and feet to yourself). Every player wants to feel safe and have their personal space respected. No drugs, alcohol, or illegal substances. We are a small family, and we can safely express ourselves to be our best selves. There is no judgment, every player will be accepted unconditionally for who they are! I will NOT treat others differently because they are a different race, color, religion, age, sex, or ethnicity than me. Choose a Positive Attitude – the best way to help yourself (and others) have the best time possible. Regardless of ability, I will try my best in every opportunity that is presented to me.
