To start your registration process, players and adult officials should download and complete the appropriate following documents and forward the completed registration form to their Team Official:

1. Membership and Player Registration Form 

The latest version of this form contains all the recent changes. Please do the following:

1. Players, or their parents, should download the Membership and Player Registration Form, or, Team Official downloads the form, makes copies and distributes copies to their players;
2. Players complete and sign the form; and have parents sign the form and send form to Team Official.

A copy of this document MUST BE provided to each player and must accompany the Membership and Player Registration Form. The Parent/Guardian signature confirms the receipt and knowledge and content of the document. Simply,

2. Provide to Parent/Guardian along with the Membership and Player Registration Form
