 Team Officials are reminded that they must provide a copy of the Player Rights & Obligations form to each player that is rostered to their team.

 US YOUTH SOCCER CARD: The USYS Member Pass/Membership Card, required for players and Team Officials, has not changed for the 2007-08 seasonal year. Supplies of these cards are available to teams through their leagues or club representatives. MSYSA requires both parts of the card to be completed and signed by players, with two glued-on photos. Photos from prior year cards cannot be re-used. A sample, which shows position of photos and player signatures, can be viewed by clicking here . Team Officials are required to complete and sign the "Membership Card" (blue) half of the card.

 

All MSYSA forms available from this site contain a date which identifies the form version. Generally, earlier versions are not acceptable. The forms are in PDF format, so you must have the Adobe® Acrobat® Reader® installed to view and/or print these forms (available FREE from Adobe®).
