US YOUTH SOCCER CARD: The USYS Member Pass/Membership Card, required for players and Team Officials, has not changed for the 2010-2011 seasonal year. Supplies of these cards are available to teams through their leagues or club representatives. MSYSA requires both parts of the card to be completed and signed by players, with two glued-on photos. Photos from prior year cards cannot be re-used. Each card must be signed, stamped by the team’s Registrar, and lamiated.  A sample, which shows position of photos and player signatures, can be viewed by clicking here . Team Officials are required to complete and sign the "Membership Card" (blue) half of the card.
