On August 16th, 2007 the affiliation packets were mailed to the listed presidents of every currently affiliated club.  Listed below are the forms included in the packet that are due back on September 30th, 2007 for the clubs to remain in good standing with the Maryland State Youth Soccer Association.

 

If your board members change through out the year, please re-fill out the Affiliate Information Form and send it in to the state office or write a letter on club letterhead stating the changes.   This will enable us to send information to the correct person.
